This scheme was
introduced by the Albemarle Centre in 1994 and has grown with each
successive year.
The Scheme
provides one-to-one support for people with Learning Difficulties
and Mental Health problems.
A contract with
Somerset County Council's LD team sees Social Workers referring
clients to the scheme for sessions ranging from 2 - 4 hours, based
on assessed needs.
Private referrals
are also welcomed to the scheme but are still normally overseen by
the Social Work team.
Most of the work
could be described as community based and involves helping the
service users with every day activities that they find difficult to
do on their own. For example, swimming, visiting the shops, horse
riding, badminton, or learning new skills.
The programme can
be adapted to meet the needs of the individual Service-user.
